Why Is There a Soft Spot on a Baby's Head? The Skull Has Not Finished

Gaps between the bones of an infant skull let the head compress during birth and let the brain grow afterwards. They close on a schedule that doctors watch closely.

What the gaps are

The skull vault is built from several separate plates of bone that grow outward from centres and have not met at birth, leaving them joined by tough fibrous membrane rather than by bone. Where several plates meet, that membrane spans a larger area, producing the soft patches that can be felt through the scalp. The largest sits towards the front of the top of the head and is roughly diamond shaped. A smaller triangular one sits towards the back, and two smaller ones lie at each side. The membrane is strong, so the gaps are not fragile, and ordinary handling and washing cannot damage them.

What they make possible

Two separate problems are solved by the same arrangement:

  • The skull plates can overlap during birth, narrowing the head
  • That reduces the diameter passing through the pelvis
  • Newborn heads are frequently visibly moulded for a few days
  • Afterwards the brain roughly triples in volume in three years
  • Fixed bone could not accommodate that expansion
  • The gaps and the flexible joints between plates allow it

Why doctors watch them

The soft patches provide a direct and non-invasive window onto pressure inside the skull and onto general health, which is why they are examined at every routine infant check. A bulging or tense one suggests raised pressure inside, which can indicate infection of the membranes around the brain, bleeding or obstruction of fluid drainage, and it is an urgent sign. A markedly sunken one is a recognised indicator of significant dehydration. Closure much earlier than expected can indicate a condition in which the joints between plates fuse prematurely and restrict brain growth, which requires surgery, while very late closure is associated with several conditions including underactive thyroid and rickets.

The measurement that matters more

Watching the soft patches is only part of the assessment and the head circumference chart does most of the work. Measuring around the head at each visit and plotting the result against a growth chart reveals whether the skull is growing at the expected rate, and a change in the direction of the line across several visits is far more informative than any single measurement. A head crossing downward through the chart lines suggests growth is restricted, and one crossing upward suggests fluid accumulating inside. That trend, rather than a single reading or a single impression of a soft patch, is what actually prompts investigation.

When they close

The timetable is well established and varies more than people expect. The small ones at the sides and back generally close within the first few months. The large one at the front is typically closed between roughly nine and eighteen months, and the range in healthy infants extends from around four months to two years, which is why a single measurement matters far less than the overall picture of growth and development. Closure means the gap fills with bone, but the joints between plates remain as flexible seams for years and do not fuse completely until adulthood, which is why the pattern of those seams is used to estimate age in skeletal remains.
