What it is
The preparation is bread, breadcrumbs, flour or another starch soaked in milk, stock or water until it forms a soft paste, then worked through minced or ground meat before shaping and cooking. Proportions vary and a modest amount relative to the meat is typical. The result is not detectable as bread in the finished food, which is the point, and the difference it makes is to texture rather than to flavour. Nearly every European tradition of meatballs, meatloaf and sausage stuffing uses some version of it.
What it does to the meat

In a meatball it is a disaster, giving a bouncy rubbery result. The same network contracts hard when heated, squeezing out moisture and fat. The starch paste interrupts the network before it forms, which is why adding it matters more than any amount of careful cooking afterwards.
Why it is not just adding water
A reasonable objection is that plain liquid would serve as well, and it does not, which is what shows the mechanism is structural. Water or milk added directly to minced meat is squeezed straight out when the proteins contract during cooking, so the meat is no wetter at the table than it would have been. The starch holds that liquid within swollen granules that survive cooking, so it is still there when the food is eaten. Starch also does the second job of physically obstructing the protein network, which no amount of liquid can do on its own.
The variations that do the same job
Many traditions arrived at the same solution with different materials and listing them shows what the common factor is. Italian practice soaks stale bread in milk. Swedish meatballs use breadcrumbs and cream. Middle Eastern versions use soaked bulgur or rice. Some Asian preparations use a starch slurry. Grated raw potato, cooked rice, mashed beans, oats and even a cooked flour and water paste all work. What every version supplies is hydrated starch, so the choice is a matter of tradition and flavour rather than function, and a gluten-free version substitutes freely without changing the result.
